Note for Windows 7 users
The Disaster Victim Recovery Form has been created using Microsoft Excel 2003.

If your computer uses Windows 7 you may initially be unable to view the document correctly. Tabs at the bottom of the screen should be visible in order to view the additional four pages of the form.

This can be corrected by selecting the following options after the document has been opened:

1. Select “View”
2. Select “Arrange all”
3. Select “Tiled”
4. Select “100%”
Online Disaster Victim Recovery Form



The Interpol Post Mortem Labels Working Group has produced a Disaster Victim Recovery Form. (Excel file )

The form is designed to be simple enough to be used with minimal instruction.

The document is based on an excel spreadsheet, when a number is typed into the first box on the form it will populate the entire form.

It is recommended that strong moisture resistant labels are used with this form; however it includes a page of labels for emergency use. 

It is recommended that the "PM number" is made up in the following format:

  • The telephone country code of the country dealing.
  • A unique reference number should then be entered, ensuring that no number is issued more than once.
  • An optional scene code consisting of letters or a number may be entered where there are multiple disaster scenes being dealt with. 

The form consists of a page of labels, plus four pages accessible by tabs at the foot of the page. When the “PM number” number has been entered, all the pages of the form should be printed off and completed in clear handwriting.

The working group has also developed a hard copy booklet, including moisture resistant labels that include centimetre scales. It also includes adhesive labels for samples etc. This will be demonstrated to the Interpol Standing Committee on Disaster Victim Identification in May 2010.
